Personal Narrative Dyspnea On Exertion - 782 Words

CC: Dyspnea on exertion. History of Present Illness: Mr. Siegel is a very pleasant 80-year-old gentleman with end stage renal disease, previously on dialysis. He is here today for a formal evaluation of progressive dyspnea on exertion that has been worsening over the last six to eight months. He states that while sitting still, he has no shortness of breath. However, with any exertion at all, he does become winded very quickly. He does note some wheeze with exertion as well as cough. Interestingly there is some association with belching, which was evaluated by GI with a negative workup. He does complain of some chest tightness, as well. Per Ira Schwartz, s MD note, cardiology evaluation of his ongoing dyspnea is not primarily related to his heart. PMH: 1. History of cerebrovascular accident. 2. Hypertension. 3. Mild dementia. 4. Depression. 5. Progressive dyspnea on exertion. 6. Gastroesophageal reflux disease. 7. Chronic kidney disease, stage 4. 8. History of prostate cancer. 9. History of mitral valve regurgitation and cardiomyopathy with last recorded EF of 45-50%. Social History: The patient is married and lives in Portsmouth with his wife. He has three adult children who are healthy. Following formal education, he has worked as a mechanic as well as owning various business on the Seacoast. He currently does not smoke, however has greater than 60-pack-year history of smoking. He quit nearly 30 years ago. He does drink a glass of liquor

The identity of the Bra Boys can be seen through their values and attitudes. They are presented through the Cody, the value of brother ship and Attitude of brother ship through their tattoos and being there for each other. In part 1 of Bra boys, the Boys had taken Jesse Pollock and other younger people on trips to surf. Values of the Bra boys is brother ship, as they had each other’s back. When 2 people had a fight with authorities in a party during Christmas, everyone got into the brawl. This shows how the value of being together in the Bra boys is strong. One major values the boys have is having fun with others. In the serious of montages, the Bra Boys are shown to be jumping on each other, setting alight each other and each them jump off the cliffs and even jumping on a bus. This shows that they highly value fun in their lives as they want all the fun. This scene plays with the light hearted tune, this influences the value of fun in the Bra Boys life and their value of belonging as they do what they want and what they believe is right. That is the idenity presented as a Bra Boy. To have fun their own way. Survival and a sense of belonging. The Bra boys values of survival is shown as they survived from various gangs, depression, fights and drugs. They had shown that they want to survive for the beach, as it is their legacy. For the Bra Boys, they showed a sense of belonging by helping each other through tough times and helping the younger generations. The Bra Boy’s identity of attitudes is shown that they have each other backs and that they do not wish to do harm. Their attitudes towards the police is hatred as they were in a fight with the authorities. However, the authorities lost but the media had portrayed them to be the victims. The police thinks the Bra boys as criminals and always suspects them, thus they have a negative attitude towards the police. The attitude towards people similar to the bra boys in the past, cold, hurt ad alone. The Bra Boy’s have the attitude of being there for those to toughen them up and be the Bra boys just as the older Bra Boys did the same thing to them. The attitude towards Ma shows their love and symbolizes what Ma is to them. Their love and kindness in comparison the language used in the other parts of the film shows how they value and their attitude towards their caretaker and the start of the Bra Boys. Their tone and voise is different in comparison to the other tones that is used. Not only that, there is something that is only presented in this scene that the Bra Boys does. (a) The types of market segments currently existing for the Hummer in the consumer market are generally men with household incomes varying between 200 and 300 thousand dollars per year. Most of these men already own two or three cars. More doctors will purchase Hummers than lawyers. This market segment consists mostly of entrepreneurs as opposed to corporate conformists. (b) The types of market segments in the business-to-business market are mostly associated with the government / military / US Army fleets. AM General, producer of the Hummer, has also signed a number of licensing agreements for the use of the Hummer name-with Converse to market rugged footwear and with Timex to produce a â€Å"Hummer† watch. 2. There are many pros and cons associated with the above market segments. For the consumer market, the demand is created by a very narrowly defined group of consumers. A specific target market is positive thing for producers of a good such as a Hummer, however it is very limiting to the overall demand of the product. In other words, the vast majority of the US population has an annual household income of much less than 200 thousand dollars and is not lead by extremely successful entrepreneurs or doctors. AM General knows which type of consumer to target, but the number of consumers in the market is relatively small. Good news for the business-to-business market! If AM General can continue to satisfy the demands of the US military, then they are pretty much guaranteed a continuously existing market. The market may shrink and expand from time to time, however the US government will continue to demand military vehicles as long as world peace ceases to exist. With the falloff in demand from the government segment, AM General should target the consumer market more heavily. With e-commerce booming and many entrepreneurs creating new innovative dot-com companies, there are men out there with money to spend! Because Hummers are more of a luxury item to own rather than a conventional form of transportation, the market would be similar to that of the Ferrari or Lotus Esprit – type vehicles. 3. AM General’s marketing mix will differ between that for the consumer and that for the business-to-business segment. For business-to –business transactions, product will meet the specifications requested by the business (their needs). Place isn’t too important. Price is pretty much lacking competition for the most part, and will only be important as to what the government can afford (contracts?). Promotion will not be a huge concern here, either. Am General will need to provide information that will make the government realize that their products (vehicles) are strong, reliable automobiles by which the government will benefit from (they will aid in military defense and development). For consumers, AM General will have to work a little harder in the promotional area. The name and image of the vehicle alone will attract customers who wish to identify with this image. However, the consumers need to believe that a Hummer can actually be driven around easily and saf ely by any consumer, not by just the military. Price will be of significance. Not many people will be able to purchase such an expensive, impractical vehicle. Place may be of importance as well. The Hummer may be more attractive to those living in certain types of terrain where they feel they can enjoy the luxury of the outstanding performance of a Hummer off road, in water, sand, etc. Does there need to be Hummer dealership on every corner in every city? No. Hummers should be available upon special request because their target market is so specific. If a consumer wants one, they will search for one. They won’t decide to drop by the dealership on their way home from work and work a deal!
